Application - Print, complete, sign and date.
Criminal Background Check - All member levels require verification that a criminal background check has been completed.
Agency Endorsement - On letterhead, currently dated and signed signifying that you are serving their department. If the agency has performed a Criminal Background Check they can include verification of that fact in their letter.
Ecclesiastical Endorsement - On letterhead, currently dated and signed from a denominational or congregational official attesting to your ecclesiastical standing.
Copy of Driver's License - Copy of state driver's license.
Membership Dues - $125.00 for US residents. All non US residents, please contact our office for current GDP pricing.
