After you download the program, install it before you try to open the Membership Application Form because it will not work in a word processing program. You'll also need letters from the chief of your department attesting to your appointment as a chaplain with the department, and a denominational or congregational official attesting to your ecclesiastical standing and other professional credentials.  Send the entire package to the ICPC Membership Office PO Box 5590 Destin, FL 32540-5590 with a check for $125 for your annual dues.
